Application for Partial Financial Scholarship

  1. Complete the application form below in its entirety. Please note there is a nonrefundable $50 application fee that must be submitted in addition to completion of the form. Within 48 hours, you will receive a link to our portal where you will complete a more thorough application with personal information.

  2. Email the following documents to our Executive Director at amie@bringyourbrokenness.com. Label the subject line “Scholarship Documents”.

    • Letters of recommendation from 3 of the following (acceptable sources and specifications below)

      • Parents/Spouse/Friend – please include your experience with their eating disorder and note your perception of her difficulty with recovery on an outpatient level

      • Primary Therapist – please include a general summary of treatment goals and progress as well as your recommendation for a higher level of care and perceived motivation of your client towards recovering

      • Dietitian – please include height, current weight, significant weight changes across treatment, any dietary concerns/difficulties and current meal plan along with your recommendation for higher level of care

      • Primary Care Physician download here – please also include recent lab work and areas of medical concern

      • Psychiatrist – please include current medication regime and success thus far in treatment as well as your recommendation for higher level of care

    • Completed consent and release of information forms for each practitioner (download here)

    • Financial documentation (at minimum a copy of your most recent tax return and six months of most recent bank statements - checking, savings, money market, etc.) Dependents must include a copy of your parent/guardian’s most recent tax return and bank statements.

    • Pre-Admission Medical Assessment (download here) . Please note that this piece must be obtained within 10 days of admission. We will process your scholarship application once all other pieces have been received and can wait on this piece until an admission date is confirmed.

3. Upon completion of application and receipt of all documents, your application will be reviewed by a board-appointed committee. Scholarship amounts will be awarded based primarily on available funds, financial need, and commitment to the treatment program/desire for change.

4. Scholarship recipients will be notified via email as soon as the decision has been made. The email will indicate how much funding has been approved for the recipient. 

 We recognize that the need for higher level of care is urgent. Our scholarship decisions are provided as soon as our committee is able to make determination.
